Dr Trevor Gray says how well patients see after surgery now comes down to optometrists and ophthalmologists working together, not surgical technique alone.
AUCKLAND, New Zealand – October 27, 2026 – Patients in the North Island who are still struggling with their eyesight after cataract or laser surgery can now ask for a second opinion from Dr Trevor Gray, one of New Zealand’s longest-practising anterior segment specialists.
Dr Gray, who founded the Auckland laser and cataract clinic Re:Vision, has stepped back from surgery following three decades in ophthalmology. He now focuses his clinical time on teaching, mentoring and providing second-opinion consultations to patients whose surgical outcomes have not lived up to expectations.
The timing matches a wider initiative at the clinic to formalise shared care between New Zealand’s optometrists and ophthalmologists, a model that Dr Gray argues has improved patient outcomes more than any single advance in surgical technique.
Dr Gray spent 20 years in the public hospital system before going into private practice. He describes the older model of care as surgeon-centric, with optometrists referring patients, ophthalmologists assessing, operating and reviewing, and patients rarely returning to the practice that referred them. What communication there was between the professions was episodic and largely by letter, organised around the episode of surgery.
“Today, the landscape is fundamentally different and unquestionably better,” he wrote recently in a column for the trade publication NZ Optics.
Much of that change rests on the growing recognition that the health of the ocular surface determines whether surgery succeeds. With a surface-first approach, dry eye and related conditions are picked up and managed before an operation rather than afterwards, improving refractive accuracy and reducing post-operative dissatisfaction. This work sits comfortably with optometrists, who can manage dry eye exacerbation and epithelial-healing concerns in their own practices and escalate cases to the surgeon as needed.
Re:Vision has been working on the referral relationships the model depends on. Earlier this year, the clinic ran a refractive surgery training day for optometrists at its Auckland clinic, led by Dr Gray alongside Dr Mo Ziaei, ophthalmologist and charge nurse Michelle Williams. Attending optometrists were shown refractive surgery technique in a hands-on demonstration using model eyes.
Dr Gray remains a supporter of the Aotearoa Charity Hospital Trust’s ARCH Virtual Hospital, through which surgeons offer free cataract surgery to patients who fall outside public hospital treatment thresholds even though their visual impairment significantly affects their daily lives. Many patients find their way to the programme because an optometrist has advocated for them.
He anticipates the partnership between the two professions will continue to deepen, highlighting structured continuing professional development, collaborative case discussion and dedicated clinical hotlines that let optometrists get surgical advice in real time.
“The scalpel may initiate the change, but partnership sustains it,” he wrote. “Our best outcomes are shared outcomes.”
